This guide clearly explains the installation of the Neo 4.5 step by step. Please read the instructions carefully before proceeding with the installation.
(1) How to figure out which version I need (GAP/ no GAP)?
The Neo4.5 chip is compatible with all V1-V4 PAL and USA PlayStation2 consoles. However there are two different Bios on the board of the PS2. In order to determine which Neo4.5 chip is the right one for you, you need to open up your console and locate the Bios chip. You then need to differentiate between:

Users with the No Gap BIOS (PAL/US PS2) versions need to solder points 13-12 on their clip as the picture illustrates below.

(2) Where is the BIOS chip located?
If you cannot find the BIOS chip on your PS2 board you may find the following links useful. They show different locations of the chip depending on your board:

(3) How to install the clipboard?
The clip needs to be pushed carefully over the Bios chip.

(4) FFC Cable Installation
Next attach the FFC Cable as shown in the pictures below. Click on each picture to obtain a more detailed description. Further make sure to either use the short or long FFC cable as shown.
(5) Chip Installation
The final step will be the installation of the Neo4.5 chip itself. 11 Wires need to be soldered onto the PS2 board as advised in the installation diagrams below:
